It's not exactly clear the ordering of refresh?
Should I always refresh Vault first? or are there conditions that change this?
What about MySQL? Should I always refresh MySQL first?

This commit updates the cluster refresh documentation. Adds some extra context for general cluster refresh and MySQL refresh procedure, and introduces a Vault refresh section.
